Band students must complete an annual physical for every year they are on the DHS Marching Band. Information regarding physicals is below. PHYSICALS MUST BE DATED AFTER APRIL 1st, 2023. There are various methods for students to get their physicals done:

Use Your Own Doctor

Download the Student Physical Examination Forms only. Please bring both pages of the form with you to your appointment. You are required to fill out the entire medical history on page 1 before your appointment. A physician or physician’s assistant will complete the physical evaluation on page 2 during your examination.

Use a Methodist Doctor

Methodist offers physicals for $20 (cash only).

Download BOTH of the following forms: Student Physical Examination Forms and Methodist Consent Form. Please bring all completed forms and signed consent form with you to your appointment.

You are required to fill out the entire medical history on page 1 before your appointment. A physician or physician’s assistant will complete the physical evaluation on page 2 during your examination.
